Can They Sue You "In Absentia?"

When a creditor seeks a judgment against a debtor doesn't the debtor have to be served with some kind of papers first? (Isn't that why "process servers" exist?). Isn't everybody supposed to get their "day in court?"

Also, don't most states have provisions where they reimburse hospitals for so-called "charity care" in cases where it is rather obvious that the person is in no position to pay the bill? (For instance, the patient applied to have welfare cover the cost - and the patient also stated in the application that he/she would file Chapter 7 bankruptcy if the application was denied). And doesn't the hospital automatically get a copy of such application (thereby letting them know the situation)?
