Gulf State

I recently tried to refinance my home mortgage, but based on a charge off reported by Gulf State Credit...I was denied my mortgage. I had no idea this was on my report...I don't know what it's for (except it's $136.00) and been denied my mortgage. This is the ONLY blemish on my report. I'm currently disputing this matter through mail, but have read all the posts about Gulf State on this web page...can anyone tell me what to do...here's the facts:
1) I've contacted the original creditor (US WEST), they tell me they don't have any record of delinqency.
2) I don't have any recolection of debt (the phone company, US West, has never disconnnected me for non-payment).
3) I don't believe I owe anyone $136.00 (an amount that I would have no problem paying if I thought I owed it to a creditor or utility company).

And now I can't make my 27-year mortgage a 15-year mortgage because of something Gulf State has put on my report.
