change date on check

In 1995 my wife issue a check to a health care company, the check was lost by the
HR personnel and was found a year later, in there attempt to cash the check my
bank refuse to honor the check because it was over the 1 year mark they have set
up for the check to be cash, the company change the date on the check and tried to
cash the check again in 1996 but the bank refuse again, they then (FHP) sent the
account to collection and they call us and told us we have to pay the money, I was
unemployed and we just have our second son I told them I have no way to pay the
money, they have now report it to the credit company and it on my credit report, I
told if they can give me the original check I will now give the a new check but the
refuse to give me my old check back, I need to write a letter to the credit company
disputing this. Is there a place I can get sample letter so I won't not sound dumb
when I write it.
