two general questions

These may be stupid questions, but I'm a little unclear on them...

1) In the absence of personal records, how can I verify the actual date of delinquency of a credit card account? The credit reports I have are unclear...they list only "date opened" and "date reported."

2) If a delinquent credit card account is reported to the credit bureaus by both the original creditor and by the collection agency to which it was sold, do the bureaus know that the entries refer to the same account?
