Form Letter

I don't have a form letter to dispute. Is there a web site I can get it from? Should I say the following in the dispute? Thanks.

"I am disputing this debt. I don't owe them anything. I never got a letter from original creditor or the agencies. They are also violating the FCRA by saying this is an open account"
