There is another possibility.
Sometimes when a medical procedure is done "out-patient"or "in-patient" the hospital submits a bill for the entire service to the insurance,gets paid, but does not account for the doctor's fees. Check with your insurance company and see what and to whom they actually paid for the surgery.
