What info required to create entry on credit report?

I have been trying to straighten out my credit report for several months now. A specific CA has been making entries on my credit report even though the debt is not mine. What I need to know is exactly what information is need by the CRA from the CA before they will create an entry on a credit report?? Do they need DOB, SS#, name, address???
