Duplicate Information on Report

Hello!
I have some accounts on my report that I have never heard of and some the accounts I know I owe are not there, Do I request proof of the debt from Equifax or from the Company that listed the debt on my report? I want to start cleaning this up but the account numbers do not match my records.
I would like to thank you guys for the help given here in advance, It is nice to know I am not the only one!
