I would suggest you send a dispute letter to the CA,file a complaint with the FTC and your State's AG under your State's statute of frauds act.Advise your
lender of what you are doing and request they set up an escrow account to guarantee payment in case you do not prevail in getting this fraudulent account removed from your records.You also might check and see what other victims of the health club are involved. There have been some class action suits in similar situations.
