Filing Suit Against Allied Interstate in Small Claims court..

OK in a quick nutshell... Sent them a verification request 60 days ago, they couldn't find anything with my signature. Sent a C&D letter 3 days later (Actually the one in Linda's briefcase). All letters were sent CM, RRR, and GC's were returned signed. I am going to file in Small Claims court for my $1,000... But I am not sure what I am supposed to put in the actual paperwork...This is what I have got from the county clerks office and not real sure what to put in this space...
