Help///Gulfstate credit

I have a charged off Providian Credit Card with a $1200 balance several years ago. They turned it over to Midland Credit management and now there is a third company on my report "Gulfstate Credit" which is on each bureau report at least twice. Is it posible that the same debt could be on my credit report 4 times (Providian, Midland, Gulfstate, Gulfstate)? What should I do? If I settle with Providian can they get the other ones taken off? HELP!
