N.Y. has so many different jurisdictions it is impossible to find any forms on-line for you. Call the court clerk where your judgment is filed and ask if they have request to vacate or request to set aside judgment forms available, or if just a letter is sufficient. If they just need a letter, then you can go to the link below and get the information you need.
