more double and triple stuff

I wrote yesterday about my Pacific Bell bill being reported twice.I was also wondering about something else on my report. A collection company (the credit store) bought an old providian account I had late last year. I agreed to their offer of putting the balence on a credit card so I could have one (a mistake I now realize). The problem is that after this I got a copy of my Equifax report and realized that I now have the same debt reported three times (two of them very negatively). The original charge off from providian is there; there is a collection account for the same amount from the credit store listed as past due on payment which is strange because I signed up for their offer when I first got it...they had reported me past due before they even contacted me...and now the new credit card with the balence on it...how do I get some of these multiple entries removed....I also am having a similar problem with a Pac Bell account as I wrote earlier...I also have a collection with PG&E ( an old bill from an address three years ago that I did not know about) for $38. Is there any way to have that removed since it is such a small amount...thanks for any help you can give me...
