credit history for employment

if a company requests a credit history for employment does the applicant have to sign a release agreeing to this? If there is a credit report inquiry done anyway does the company have the ability to continue to request the report many times during the employment period legally?
If, during the employment period the company request a credit report and there is adverse action taken(termination) and part of the decision for termination is based on this report does the company have to notify the employee within 3 days of the decision. This is how I read it in the Fair Credit Reporting Act.
