I went through something similar with an insurance company I represent. I sent a letter to the CRA stating that I had never signed a release or Virginia privacy notice (VA has a separate law) and that I was going to sue them because they had allowed the inquiry to take place without the proper authorization forms. CRA wrote back and said that I had signed the forms when I applied to represent this particular insurance company (Jackson National Life Ins).

JNL was being sued for the actions of some of their agents, and in a CYA action they were trying to amke it look like they had done their due diligence when they hired all their agents (they had never checked out anything except whether I was licensed).

I wrote JNL and requested copies of the appropriate docs - they never answered me. I let it go because there was no damage done. I no longer represent them, however ;)
