Please HELP!

I have this collections agency that calls me at work and at home daily. It is for a Dentist bill for $485 that I thought insurance was going to pay for at the time. I did not have the money to pay for it or else I would have. So I after the Dentist office quit sending me statements I forgot about it. It has been well over a year ago. In Dec.2001 a Debt collection agency contacted me wanting me to pay them which I probably could have taken care of until I found out they were charging me 50% more of what the original bill was so now I owe $727.50. They have been harrassing me at my work demanding I give them a check by phone to settle this matter. I called the Dentist office to settle the bill the girl there said they would just send the collector the check. Does that mean that the collection agency probably ownes it now?

I tried to talk with the collection agency and see if they would agree to accept $100 over the original balance. They said they would apply that to the balance. My questions is: After paying the original $485 bill can they report Bad or unpaid on my credit report for their fee?

Which is all I feel I have to pay especially for the harrassement I've been through.
Are there any regulations as to how much they can charge you over the original bill?
I am also in the near future going to be purchasing a home can this hurt me, or keep me from getting a loan?
