Banana Republic CC account sent to a collection agency

My mother opened a Banana Republic card last year so that she could receive 10% off of her purchase. She never recieved the card and thought she paid the bill but is now unsure. Anyway, months later she has now received a call from a collecton agency. When the agent tried to verify our address my mother realized they had 16th Street instead of 19th street where we live. 1) How come we never received a call from Banana Republic in all of this time? 2) How come B.R. sent the account to a collection agency without first notifying my mother by phone of what was going on? I have several credit cards that I'm paying off and I received letters as well as phone calls when I had trouble paying a while back. 3) Is anyone in the wrong since they sent the card and the bill to the wrong address? What can my mother do now? Will thi have to remain on her credit history considering B.R. made a mistake with our address?
