YOu mean removeal from your credit report?? If the debt is this old, it should be removed, no problem. I never use a special form for this, I just used the little form they send with your credit report.

Since you have been denied for credit, go to the credit bureau's websites, and check the bos that says you have been denied credit, and request that they send your report by mail. The dispute form will come with it.

Just dispute it as "out of date" be sure you keep you copy of the credit report - you will probably need it.

If you are experienced with online stuff, you can do it right online, at their websites.
