I dont remember what the o5 means but if it lists Closed By Consumer, that means that the account is closed and paid for. It doesnt mean that the debt was not verified. If that was the case, it should have just dropped off. A lot of times if the account is closed but was never paid for, it would list like Account Closed - Account Transferred or Sold.
