CAN CREDIT AGENCY PUT BACK ON REPORT

I have a charge off listed on my account, that I am paying on monthly with the creditor, this is a goverment charge card, that is the only reason why I am doing this. My question is, it is showing on the report as just a charge off and not even saying paying monthly as agreed. I disputed this with Equifax, and Trans Union. This was back in Sept. They both removed the item from my report. In Oct Equifax put it back on showing the lower balance that I owe, What I want to know is, can they put it back on my report after it was removed? I thought they had to send you a written letter saying it is being put back on the report.
