Try sending a certifed rrr letter asking for a complete account history FROM CHASE. I'm sure you have your own.

Once you get that, or if you get another form letter, take all your stuff and file a complaint with your state's AG, your state's banking commission (or whatever they call it), the Federal Reserve, Office of the Comptroller of the Currency, the BBB, and last (and least) the FTC.
