April 15, 2002 Certified Mail Number:

Dear Equifax,

This letter is a formal complaint that you are currently in violation of the Fair Credit Reporting Act.

On March 15, 2002 I filed a dispute for inaccuracies on my credit report. Included in this dispute letter was a request for the procedures used in the verification of these inaccuracies. This information was to be provided to me within 15 days of the completion of your investigation as required by the FCRA. I have yet to receive the information I have requested and that you are REQIRED BY LAW to provide.

In this letter I have also requested contact information. Under the FCRA you are REQUIRED BY LAW to provide this information upon request, yet have failed to do so.

As my letter was partially acted upon ad evidenced by the results of your investigation dated April 15, 2002, I can only assume that you have not provided this information either willfully or through negligence.

To eliminate any possibility of an honest mistake you have 10 days to provide me with the information I have requested. As a courtesy I have provided copies of all relevant documentation regarding this matter.

You are currently in violation of the FCRA, failure to provide the requested information in the allotted time WILL result in my filing complaints to the Federal Trade Commission and my states Attorney General’s Office, and possible civil action
