You want a complete accounting of the debt from the date acct was opened till present. Proof you applied for the acct or contract with your signature. Agreement between the OC and CA assigning, sellin or transferring the debt to the CA signed by the OC. All three are required for proper validation.

You also want contact information to the OC. So you can to contact them for the same info to verify the CAs claims.
