Questionnaire validation only sent back

I just received back from Action Collection Agency the questionnaire only. this debt is 6 years old, it is from a medical bill around 88.00. should I send them a letter stating they need to provide the requested validation which was sent with the letter? any one could have obtained the info they gave it is all over my credit report!
