CA - Fraud Statement ?

Someone used my daughter's SS# and DOB to open a fraudulent cell phone acct with Sprint in a totaly diff state.: Discovered this while doing a routine C Check. Sending a valadation letter to the CA. They have a
form fraud statement to fill out requesting a copy of the SS card and Drivers license to be attached. Should I photocopy and " write" copy over the signature so it can not be used and snd it ?
