sears credit card charge

The question is what do we do? Last August my wife paid cash for $68.99 worth of merchandise and was approached with an offer to open an account at Sears for 10% off of the purchase. She filled out the application but did not receive a card or any other correspondence until a bill was received for the items that she paid for that day along with late charges and fees that brought the total up to $233.55. After she called about this she received another bill with the extra fees credited but the original charge outstanding. They also sent a copy of a slip with the items and her signature that seems to verify that the items were not paid for in cash as my wife claims, but were acutally charged on a credit account. The account numbers on this slip do not match the card that she finally received last month with the last bill. The key question here is: "Does Sears actually have people make out a store promotion credit application, and without going through a credit check or acceptance procedure, allow them to charge a purchase on a mere application?" A "No" answer to that question would bolster my wife's assertion that she indeed pay cash and does not owe Sears again for the merchandise. At this point the fees are beginning to mount and she has been unable to reach a human being to talk to about this. Please help. Thank you.
