The easiest way is to go to the website of the agency where you got the report (actually you may want to dispute it with all 3 if they have it listed).

Go to their dispute section. The report you bought should have a report number or confirmation number on it. Use this number to access your report online.

When you see your report, scroll to the entry you want to dispute. When you select it a window should come up asking the reason of dispute (i.e. never late...etc)choose the apropriate response. You can also add a comment if you want (I wouldn't bother with that).

Within 30 days you should be notified by mail of the results. If it is deleted you will get a new credit report saying it was deleted and showing you that it is no longer being reported. I don't know if they send you a full report for diputes that are all verified (I've only had them all deleted or some deleted and some verified on the same dispute...so I've always gotten a new report).

Since it's paid and 4 years old there is a chance that the creditor has disposed of the record and won't be able or bother to verify it. If it does get verified you can always try again later ( I'm not sure how long you should wait to dispute the same item, but wait at least 3 months before you dispute anything else).
