Address flag?

my husband went to get a cell phoen today and they ran his credit and it came up with a A1 credit history-but with a flag on the address-why would that have happened>? They are now making him bring in proof of his address so he can get the phone. I don't understand this-all his bills come to this address-why would there be a flag on it? Is there any way my credit could have gotten intermingled with his? They did NOT have my SS # to run, so I don't know how that could have happened, I am probably being paranoid-and my credit history sucks-I was thinking that it could possibly be because we live in an apartment and the apartment number he uses on all his bills is different from the apartment number that some of the utility bills and the city has on file-is that possible?
