Duplicate Adverse Entries

A credit card account was "charged off" by the XYZ Bank and it
appears on my credit report.

Four (4) years later, the "charged off" account was sold and then
turned over to a collection agency who reported it a second time
on my credit report. It appears on my report as" "Originally XYZ
Bank Account".

A year later, the "charged off" account was sold again and turned
over to another collection agency who recorded it a third time on
my credit report. This time, however, the collection agency
deceitfully reported the "charged off" account as though it were a
new account.

I've been told that duplication of an adverse entry is illegal.
Where could I find this in writing???
