seek advice on collection letter on account that is very old

actually this is my wifes old account with a workoout gym.
she cancelled the account 12 yrs ago. no details in the collection letter but they had her old name on it(prior to our marriage 9 yrs ago.) and it was sent to her mothers address.
we think this suspicious as she is not aware of or has never been made aware of any money owed till now.
is there recourse?? we think we should notify local a=state authorities and or the FTC.
