concerning your "Old" Collections..
I live in Washington State. My wife just recieved a collection notice using her old name and sent to her motheres residence.
It is from a collection agency and makes reference to an old membership that was cancelled 12 yrs ago.
She has not recieved anything prior to this notice.

I suspect this collection agency picked this one up in a bundle and is now trying to collect.
there is little info in the letter. No specific information short of the mention of the Gym and the amount.
