MEDICAL BILLS FROM 7 YEARS AGO...HELP

I have recently received a call at my work place from a collection agency demanding a $400 down payment and $100 a month on medical bills that are 6-7 years old. The total of the medical bills is a little over $3,000. I never heard a word about these bills until now. The hospital never contacted me about this, I just assumed that my insurance took care of it. What do I do about the frequent calls? I can not afford to pay this, I only make $1,200 a month and there are 5 of us in the family. Is is legal for me to send them a letter telling them that I can only pay $20.00 a month until the it is paid in full and for them not to contact me at work? I live in Kentucky.
