another credit question

This is in regards to my last question. My Bally's account was turned over to the Collection Agency (Genesis Financial). After looking at my credit report I see an entry for Bally fitness as a charge off, then I also see an entry for Genesis financial which is the CA. Why are there 2 entry's for the same account on my credit report?? Can a collection agency report to the 3 credit companys?? It looks I have double the debts, This is not right, right? Thanks,
