Time to be notified of a debt

Question: If someone dosen't tell you a debt exists, when does it not become a debt?

I went to the hospital 20 months ago. My insurance paid all calims filed.

Now, 20 months later, a doctor is requesting payment. The insurance will not pay him because he waited too long to file the claim.

I am certain that if someone dosen't inform you of a debt before 12 months passes, no debt exists.

Can anyone tell me what Federal Law or Act covers this item and where it can be found in that Law of Act?
