And if it is a fake, consult the district attorney in the largest city nearest your residence that is not your own city for advice regarding which authority to notify first. If they are using a local telephone exchange, there may be collusion with your local authorities, you really can not be too careful if the item is counterfeit AND the bodies involved are anywhere near your living space.

The document may be genuine, even though you were never aware of any court action against you. Lord knows many are the ways and means of laboring to serve a legal action while ensuring that somewhere along the line you'll have proof that service was refused or ignored, whichever best suits the litigant's purpose.

A counterfeit document of this kind, however, racks up a chain of criminal offenses beginning with mail fraud and extending from there to federal and local statutes.
