You certainly seem to have had more than your share of problems. What he wanted was the form for obtaining discovery from the plaintiff. He mentioned that the only "proof" of claim in the complaint was an affadavit. I still do not understand how or why you could think a notarized affadavit from you on whatever subject you thought appropriate could possibly be involved. As a matter of simple precaution, and not to single you out, it is NEVER a good idea to send anyone personal information, and it is a "red flag" when someone requests it.
