I had a similar problem with a doctors office. One big question, is was the insurance HMO or PPO? If it was HMO, you are contractually responsible for the copayment and nothing else. The hospital has a contract with the insurance company that states they will accept insurance payment, your copayment and "eat" the rest. If it was a PPO, you are responsible for the deductible/percentage. But, your arguement should be that they had an internal error that lasted 2 years. You have changed insurance, employers, and states, and are unable to contact the insurance company to get their explanation on what was paid, and what was not. Do ask for the hospitals detailed accounting balance. When they billed the insurance, how many times they billed the insurance, how much the insurance paid, and when they tried contacting you. If they did not file with the insurance company withing the filing deadline, you are not responsible for any charges. This method worked for me. Hope it helps you.
