I went in person to the local Equifax affiliate and argued myself blue in the face. They called the Dr's office while I was standing there, and the doctor said they had my SSN on file (it was the identifying number on the insurance card). I was furious, and tried to tell them that proved nothing, that my insurance card had been stolen along with my wallet and identification.

I guess it was a strange situation, and they didn't believe me. That account stayed on my report for almost another year. After a while I disputed it by mail (the little forms Equifax sends with your credit report) and it was deleted. Fortunately for me, it was never sold to a CA, and I have heard nothing more about it.
