Dispute Letter

First I'm in AK the check was written in AZ the new collection agency is National Check Control. The check was written to Best Buy. The originating CA was Equifax. Then they sent the account to a lawyer, Then the account went to Certegy Payment Recovery Services. Now they tell me the account is as National Check Control. They have been sending the statements to my parents address and I did not get them till just recently. The check was written in 1999. My mom emailed me a copy of the letter National check control sent and they say I owe twice as much.
I need a good dispute letter to send them and a letter to send to to Equifax because they are still reporting it and it is on my credit report.
