New negative info on report...

I try to check my credit report monthly as it seemed to have helped. Last night, as I was checking I found a new negative debt that supposedly I owe, which I have no clue as to what it is, so I disputed it. Anyway, my question is on the report it says the last activity was 1996 but they just reported on it June 2002. Is this allowed, waiting 6 years before reporting just as it's about to run out? Also, as I mentioned, I check my report monthly and when I checked it in Sept and October it wasn't there.
