Time frame to Notify CA of Pymnt?

My insurance company FINALLY paid an old medical bill from '98. The hospital states that they never received the payment; however, my insurance company states that a bulk check was mailed on 10/31/02 and CASHED 11/8/02. The hospital is repeadetly asking for other various information and dragging their feet at clearing this up. Is it correct that they have 30 days from payment date to notify their collection agency, RMA! Also, this was my insurance companies error, not mine, how to I get this off of my credit and not just marked as paid? Thanks in advance!
