With the credit report in hand contact your local police department to file a fraud report have them include the information about this account into the report. Get a copy of the police report.

1. Call the Bank - get an address for reporting fraud, with further instructions for what they require from you other than copy of police report. Send these things, with letter explaining your situation. Ask them to investigate & forward you a response of their findings.
2. Contact the Collection Agency doing the same. As well as forwarding copy of your letter sent to the Bank.
3. Contact the CRA's doing the same. As well as forwarding copy of your letter sent to the Bank & collection agency.
4. Contact the FTC & report the fraud to them.

Keep track of your contacts with each company and follow up any new leads that you get as well. They need to know that you are serious and willing to go this far. If after you exhaust this much pull in a lawyer for assistance. Good Luck
