Clearing Up Credit Report

A little advice, please, to cleaning up my report efficiently...

I live in California and have many charge-off and collection accounts from cc's on my report(s). They are fairly old (2-3 years min.) In reading various posts it sounds the best way to get them removed is to dispute them as "not mine" with the CRA? Is there a sample letter to do this? Being it's suggested to not do too much at once, not wanting to draw attention, what is a good rule of thumb (# wise) to dispute at once and is it better to focus on the large balance items? Lastly, what to do if the OC is on the report along with the CA that bought the account?
