It's simple, they are looking for YOU to validate their claim.

Answer all the questions truthfully, your name (if it is spelled correctly) address etc.

If you are in the possession of complete records of everythng relating to this account, then you can answer all their other questions truthfully,

Otherwise say "no knowledge",if they ask questions relating to other matters, say "not applicable".

Make sure you return a copy to the court also, and keep a copy for your records.
