CA settlement sample letters???

I need to find a sample letter to present to a CA stating that "I am giving them XXX dollars to settle my account. In return I want my credit report to say XXXX and this payment is accepted as paid full and nothing will not be sold to another CA." Any help is much appreciated!!!
