Help, Please. Financial Disclosure form due in 3 days

I posted this a couple days ago, and nobody replied. Is there a problem? If you could just give me some input on self-employment income, with respect to this situation, I'd appreciate it.

State: MN
Eek! This weekend, I received a UCF-22 Financial Disclosure Form from the courts, and I have until Saturday to complete and return it. I'm self-employed and my only bank account is for my small business, which not doing too well. The form asks about exempting after-tax earnings and also for my bank account number. I should file bankruptcy, but don't have the money right now, and can't get even a telephone appt. with a volunteer lawyer this month.
1. If this goes through, what happens next? How could they garnish my self-employment income when it fluctuates all the time, and business expenses come out of it too?
2. Is there another step after this by the creditor? Would they have to file something else to get at my bank account (in which I keep a very low balance)?
3. Would filing bankruptcy quash this, and how much difference would the timing make (before or after)?
4. When it asks for total wages, salary, or commissions per pay period, can I put "N/A", or do they want some sort of monthly or weekly average income? Or would the business income be put on the "income from any other source" line?
Or is all this just a way of the creditor trying to find out if I have anything worth taking?
Any help you can provide would be appreciated. My stomach is in a knot. Thanks!!!
