Try sending your off in writing instead, phone calls just don't work. This thing is 3 years old, the doctor has already gotten his tax break for the bad debt, he might actually take 50% by now.

If they still won't accept 50%, then make your offer of payments on YOUR terms - like getting this off your credit report (if its there) in return for payment.
