Insurance wants money back! Suggestions please!

Hello, I have a situation and I hope someone here can enlighten me. Back in 2000 I had surgery done and the insurance co. I was with paid for part of it. (around $4100) Now nearly 3 years later they write to me and tell me they want that $4100 back because they claim I was not covered. I of course ignored it and just recently I received a letter (I suspect it is computer generated, not personal) from Accent (an insurance CA) simply stating that the insurance is demanding a so-called refund for the $4100. I am ready to send them a letter that states that it is the insurance co.'s responsibility to know if I was covered before they even sent me any money and that I am not responsible to pay their so-called refund and by no means will I pay for it. I live in Michigan, the insurance co. is based in Nebraska. What I'd like to know is are my rights the under the FDCPA the same as with other debts? Anything else I should know? Any help is appreciated because from past experience I have seen there are a lot of loopholes when it comes to insurance and these companies always seem to get things their way! Sorry for the long post.
