Same charge listed 3 times on CR from different companies

I was reviewing my credit report and noticed that the same credit card charge-off was listed 3 times under different company names as follows: (1) FirstChicago, (2) FirstUSA, and (3) Unifund. All three have the same amount listed, same "last activity date" of 09/95, but #1 & 2 have 11/02 listed as date to come off report and #3 has 02/04 listed as date to come off. This credit report is an old one from last year, but it should pretty much be the same except for the ones scheduled to come off 11/02. First, how can this one charge be listed 3 times under different company names and secondly, how can there be a different date to come off report for the third one? Anyone that has any insight as to how this could happen, please advise. Thanks!
