OPN 05 What does this stand for?

On one of these accts. has OPN 05 and a no. history stating 5. How can this be as I never promised to pay or have paid on this. It wasn't on a cr report from 2001 yet all this shows up on a report from 2003. And where does it tell you when it should drop off your report? So many questions too short a life.
